We are a New York State Department of Health Licensed Home Care Services Agency, specifically established and designed to provide high quality, convenient, flexible and cost effective personal care and nursing services to Sterling Glen residents and Seniors who live in surrounding communities. Sterling Glen Care at Home was created by the owner of Sterling Glen to ensure the highest quality of care. It is a home health care agency that builds on our years of eldercare experience. Through our strong local affiliations with exceptional area health care systems, as well as our ongoing commitment to quality assurance, Sterling Glen Care at Home is a home health care services agency that strives to continually expand both the quality and scope of services and opportunities available to our clients. Upon enrollment in any of our home health care services, a thorough assessment is conducted by a Licensed Nurse in order to determine your specific care needs and special requests, from which a personalized care plan will be developed just for you. Your physician and your family will also participate in designing a plan for your home health care services to ensure that all your needs are met. Determining your Needs
Once you choose Sterling Glen as your Home Care Agency all clients immediately receive:

• A thorough assessment of functional psychological and sociological needs
• A detailed Plan of Care is developed
• We send the Plan of Care to client’s physician for approval
• The Plan of Care is reviewed during a scheduled Family Care Plan Meeting

Quality Care Begins

• A customized Plan of Care is implemented with the physician’s approval
• Sterling Glen Aides assist the client on their road to wellness

Ongoing Protection
Sterling Glen protects all clients on an on-going basis through the following:

• Monthly supervisory visits by a Nurse who is on call 24/7 for emergencies
• Continuous in-service training of Sterling Glen Aides and constant back-up coverage
• Close contact with the client’s physician and family
• Regularly scheduled Family Care Plan Meetings
• Sterling Glen’s Quality Assurance Program headed by our corporate Director of Wellness

• Registered Nurse on call 24/7
• Assistance with bathing, grooming, dressing and walking

Please ask for a detailed listing of our services and prices. After your first thorough geriatric assessment by our Nurse, the price of your customized service package is based on the suggested Plan of Care.

The Best Caregivers

We guarantee that all Sterling Glen Aides meet the following criteria:

• Certified as a Home Health Aide (HHA) or Personal Care Aide (PCA) by the State of New York
• Complete new hire orientation and continually attend educational programs including fire safety, emergency procedures, client rights, and infection control
• Pass pre-employment reference checks, drug screen and thorough criminal background check—no exceptions
• Receive regular supervisory visits by a Nurse
• Supervised by a Staffing Coordinator who ensures all shifts are covered

What Does It Mean to be Licensed?

• Personal care services are provided under the direction and supervision of an RN.
• We are periodically surveyed by the New York State Department of Health to assure compliance with all regulations.
• Various insurance plans cover our services. We will assist in applying for benefits under the home care portion of your long-term care insurance policy.
• Our Quality Assurance and Improvement Committee includes a licensed physician and a Registered Nurse.
